Medical Overview

All employees have access to an HSA-eligible plan and two copay plans. If you reside in Maricopa, Pima, Pinal, or Coconino Counties in Arizona, you also have those same three plans available in a performance (narrow) network.

Which Plan is Right for You?

Our medical plans vary by provider network, how much you pay each paycheck and what you pay when you get care.

Gold (750) POS Plan

This plan has lower out-of-pocket expenses and higher paycheck deductions than the other plans.

  • Each covered person has a deductible of $750 with a $1,500 cap for the family.
  • This plan has copays for office visits and prescriptions.
  • This plan is NOT HSA compatible.

Silver (HSA) Plan

This plan has a lower deductible and out-of-pocket maximum, and higher paycheck deductions than the Bronze (6350) POS Plan.

  • If you enroll in employee-only coverage, your deductible is $1,750.
  • If you cover your spouse and/or child(ren) on the plan, you have a combined deductible of $3,500.
  • The individual deductible does not apply when you cover family members.

This plan is HSA compatible.

  • This means the plan meets the requirements to allow participants to contribute to their own tax-free Health Savings Account (HSA).
  • Your HSA lets you save pre-tax dollars to pay for your eligible health care expenses anytime, even in retirement.
  • Be sure to review the eligibility requirements to see if you are eligible to contribute to an HSA. If not, you can still elect Silver (HSA) POS plan but should not contribute to an HSA.

Bronze (6350) POS Plan

This plan has a much higher deductible and lower paycheck deductions compared to the other two plans.

  • Each covered person has a deductible of $6,350 with a cap of $12,700 for the family.
  • This plan has office visit copays but prescriptions are subject to the deductible.

This plan is NOT HSA compatible.
